12-Month Weight Loss and Adherence Predictors in a Real-World UK Tirzepatide-Supported Digital Obesity Service: A Retrospective Cohort Analysis

Adherent patients lost an average of 22.6% of their body weight at 12 months

The Juniper UK digital weight-loss service combines weekly tirzepatide with multidisciplinary team support of health coaches, prescribers, and dietitians delivered through an app-based platform. This retrospective analysis examined 19,693 patients to assess both the magnitude of weight loss achievable in real-world conditions and the behavioural patterns that predicted who stayed on the programme for a full year.

Among adherent patients (n = 5,322), who had at least 10 medication orders and a 12-month weight submission, outcomes were substantial. 

Patients meeting the 12-month adherence threshold achieved a mean weight loss of 22.6%.

Sustainable weekly engagement determines long term weight loss

  • Consistent weekly weight tracking was positively associated with both 12-month adherence and total weight loss.
  • Health coach communication frequency showed the same pattern — regular health coach interactions increased the likelihood to persist and lose more weight.
  • Patients who lost or tracked their weight at an excessive rate in the first month were more likely to drop out within 12 months, suggesting that unrealistic expectations and unsustainable pacing are early warning signs of disengagement.

Comparison to other real-world evidence

  • Ng et al. reported 16.5% mean 12-month weight loss for real-world tirzepatide-pharmacotherapy without integrated lifestyle coaching.
  • Johnson et al. found a 17.0% (non-engaged) versus 21.5% (engaged) 11-month weight loss in a comprehensive digital weight-loss service, supporting the synergy between medication and behavioural adherence.
  • Almost all adherent patients (95.5%) lost at least 10% of their body weight within their first year, with an average loss of 22.6% - the highest real-world weight loss outcome reported in Australia to date.

What this means for digital obesity care

The findings reinforce a central tension in pharmacotherapy-integrated digital weight-loss services: they deliver clinically significant weight loss. Patients who engaged in weekly weight tracking and regular health coach interaction achieved 22.6% mean weight loss over one year, underscoring that consistent but not obsessive adherence drives successful weight loss.
